STAR (Medicaid)
Congratulations on your pregnancy! It is important that you visit your OB/GYN on a regular basis. As a Cook Children's Health Plan member, you will get a $50 gift card for one prenatal and postpartum visit. You can print out this form and bring it to your doctor's visit. When your doctor signs the form, mail it back to us for the gift card. During your pregnancy, you will need to visit your doctor often. Below is a table of the prenatal visits needed.
After you deliver your baby, you will need to see your doctor for a postpartum visit. You can see your doctor as often as you need while your Medicaid is still active. When you deliver your baby, call 2-1-1 and Member Services to let us know.
