STAR Provider Directories

The STAR (Medicaid) Provider Directories list Cook Children’s Health Plan doctors and other providers. You can contact Cook Children’s Health Plan Member Services to get a paper list of doctors mailed without charge upon request within 5 business days.

Your Primary Care Provider (PCP) will also arrange any care you need from other doctors. Please call your PCP to make an appointment as soon as possible.

How to choose your Primary Care Provider

A Primary Care Provider provides or coordinates all of your or your child’s health care services. A Primary Care Provider can be a doctor, nurse or clinic that gives you most of your health care. Our Primary Care Providers are listed in the “Primary Care Provider” section of the directory. They are listed by city and then by the kind of provider. Pediatricians see children. A family practice doctor sees all ages. Internal medicine doctors see family members 18 years old and above. Age limits are listed for some providers to help you pick one for you and your child.

You will see each Primary Care Provider is listed as Open, Closed or EO. Open means they are accepting new patients, closed means they are not accepting new patients and EO means they are willing to see existing patients only. You can pick any Primary Care Provider listed as open for yourself or your child. Each covered family member can pick a different Primary Care Provider within the Cook Children’s Health Plan network.

There is no limit on how many times you can change you or your child’s Primary Care Provider. We make Primary Care Provider changes the same day that you call us.

What is a referral?

Your Primary Care Provider is the only doctor you need for most health care services. If you have a special health problem, they might ask you to see another doctor or have special tests done. This is called a referral.

You do NOT need a referral for:

  • True Emergency Services
  • OB/GYN care
  • Behavioral Health Services
  • Routine Vision Services
  • Family Planning Services

If you have questions or need help finding a provider for these services, you can call Member Services at 800-964-2247.

OB/GYN care
You can pick an OB/GYN (doctor for women’s health) without asking your Primary Care Provider or getting a referral. You can pick any OB/GYN from the directory that is taking new patients.

You can see your OB/GYN for:

  • One well-woman checkup per year
  • Care related to pregnancy
  • Care for any female medical condition
  • Referral to other specialists

Mental health and substance abuse
You do not need permission or a referral from your Primary Care Provider to receive these services. Call Beacon Health Strategies at 1-855-481-7045, 24 hours a day, seven days a week.

Vision care
Glasses and routine vision services are covered by the Cook Children’s Health Plan. You can get services from any vision provider listed in this directory. You do not need to ask your Primary Care Provider or get a referral. If you have questions or need help finding an eye doctor near you, please call National Vision Administrators (NVA) at 1-877-366-2576.

Family planning
You do not need to ask your Primary Care Provider for these services and supplies. If you are under 21, you do not have to ask for your parent’s permission before getting help. You can go to any family planning provider, in or out of network. You do not need a referral from your Primary Care Provider. You may get family planning services from your Primary Care Provider or other provider listed in this directory. If you have questions or need help finding a family planning provider, you can call Member Services at 1-800-964-2247.
