What if I am too sick to make a decision about my medical care?
If you are sick and cannot make decisions for yourself, you can have someone make decisions for you by filling out a form called an Advance Directive. For more information on Advance Directives, call CCHP’s Customer Care Department at 1-844-843-0004.
What are advance directives?
An advance directive lets you make decisions about your health care before you get too sick. This form allows you to tell your family and your doctors what kind of care you want if you are not able to and if you want anyone to make decisions about your treatment. These are the two most common types of advance directives:
- The Living Will explains your health care wishes in the case that you are terminally ill or injured and cannot make decisions.
- The Durable Power of Attorney for Healthcare allows you to name someone to make your healthcare decisions for you in you cannot make them.
An advance directive can be cancelled at any time.
How do I get an advance directive?
